There is a original Q3 level called The Place of Many Deaths (q3dm4) which I assume the title was borrowed from. Maybe Morbid Angel are Q3 fans :]

I would be guessing that the title was taken from the Morbid Angel song of the same name, but Heretic (the album it is on) was released after this map.
