Despite of some of the settings, mentioned in the description of the YouTube video with the run, changed for more comfortable and progressive run, it stays a REALLY impressive achievement, in my opinion.
I'm truly glad to see that after more than 20 years people still improve their skill in the game and accomplish higher and higher goals ✊ The history is being made! 🙌
EDIT: The thread has been transformed to the one dedicated to all the new world records in the Q3A + Q3TA speedrunning, rather than just one record. There has already been a lot of competition at least indirectly related to the very first record reviewed in this topic, so I thought it would be a good idea to rename the thread to let cover all the new best runs.

Might have been sub 40min, but just how many games and hours of practice to reach that :)
I remember Raisy catching opponents with rails in the lower tunnel on QC's DM6 right after they spawned. I think id Software even had to decrease the probability of spawning down there because of these particular case series, hahah)
In his very first deathless Nightmare! run video Raisy stated that it took him 287 tries, so yes, it definitely required quite some time and work, ahah)
Btw, Raisy's just dropped another world record video, only couple of hours ago. This time it's not deathless, but it is also on the Nightmare! difficulty in 34:17.99 with loads and 33:40.39 without loads, and the run is dated by January 22, 2023:
UPDATE:
Raisy's recent record in the Any% Nightmare! category (with deaths) was broken on the 24th of January 2023 by a Russian player FeaJz! The run was done in 33:02 LRT and 33:40 RTA.

Today Raisy dropped a video on YouTube with one even more world record, beating FeaJz's former record, in the category Any% Nightmare! with deaths! 🔥 The run is dated by today, the 1st of February 2023, and is done in 32:37.52 LRT and 33:16.02 RTA.

Today, on the 2023.03.01, Fearz broke Raisy's former world record again, establishing the new one in the category Any% Nightmare!! The run is done in 32:10 LRT and 32:49 RTA.

Raisy just broke Fearz's record one more time, I had a chance to watch the stream in live on Twitch! The run was done in 30:56.08 LRT and 31:34.86 RTA (still Any% Nightmare!).

Raisy established another world record on the Nightmare! difficulty in the new category — "No jumps". I might be wrong, but I think he managed to do the run in 36:33 LRT (not sure about hundredths of seconds). And then he improved on the same stream, and made it 36:27.99 LRT and 37:07.88 RTA.
EDIT: I went back to watch the recorded stream, and here is the story: Raisy uploaded on YouTube an unlisted video with the first run in 36:33.60 in the No Jump category on the 31st of January 2023.
Then yesterday (CET), on the 2023.03.02, he managed to break his own record, making the new run done in 36:27.99 LRT and 37:07.88 RTA.

Yesterday, on the 2023.04.04, Raisy made a second improvement in the category Nightmare! No Jump, therefore the new world record is now 33:06.15 LRT and 33:45.18 RTA. This is around 3 minutes and 21 seconds improvement over the previous record.

I started off the run by restarting the map vs Crash so that she immediately spawns into the arena instead of at 12 seconds (like she otherwise would,) making it faster to complete the first map.
The command is: map_restart "0"
I used this method of force spawning the BOTs many times throughout the run so that I can get to fragging faster.
Against Ranger on the 2nd map I respawned until I got the optimal starting position. I did this on most other maps, as well.
I did something similar for "DM18: Space Chamber" to start the map off by getting a spawn telefrag.
I turned off the BOTs' chatting, so that they don't idle around wasting time.
